MBS & Skilled Nursing Facilities
The focus is to provide care closer to the patient to reduce risk of infection when moving patients to radiology and to reduce cost. This has the added benefits of improving access to care. In particular the MBS or cookie swallow exam has become an important exam and will continue to increase in importance with the aging of the population. Moving a patient from a skilled nursing facility to a hospital is certainly expensive, but as we are now all too aware of, increases the risk for infection not only for the patient but for all residents in the SNF. Insight Agile is so, well, agile, that it can allow these exams to be performed inside the SNF, shrinking cost and shrinking risk.

Insight Agile makes possible modalities with considerably better patient access as shown the top image. This improved workflow make the exam more comfortable and quicker.

Neonatal
We are particularly proud to be possibly the only fluoroscopy system FDA cleared for neonatal fluoro. Insight Agile is so portable that we can perform fluoroscopic imaging within the incubator.
The benefits of performing fluoroscopy inside the incubator has been written about for 20 years, but until now, nobody has been able to accomplish it. Insight Agile can perform fluoroscopy up to 30 fps or as low as 2 fps on an ISO 2430 DR detector. See this study, or this one for additional information.


Exams such as Upper GI, VCUG and tube placement can easily be performed within the incubator on a system designed for neonatal use instead of transporting to radiology and using an adult system. This will lower the dose and reduce risk.
Imaging Centers, Surgery Centers, Otho and even Vet
Many markets utilize surgical c-arms not because they are the right modality but because until now, they were the only modality. Insight Agile now makes it possible to create modalities which better fit niche markets.

ALARA, it’s built in
Many modern fluoroscopic systems utilize pulsed fluoroscopy as an attempt to reduce patient absorbed dose. To accomplish this, the system pulses a large dose then turns off for a short period of time and this results in a stroboscopic effect of motion reducing temporal resolution. This was the only way to reduce dose for the past decade or so. The downside is that if the radiation pulse was 10ms long and system set at 15fps, then the system acquired 150ms of data out of 1000ms. This means that 85% of the data was not sampled, essentially thrown away. Additionally, the rise time and fall time of the radiation contribute to patient absorbed dose but not to the imaging. To simulate smooth motion most of the system utilize some form of frame averaging as shown below. By utilizing the latest in detector technology and image processing and combining with continuous fluoro and acquiring at various frame rates we are able to reduce dose by 70% depending on settings.



Imaging Engineering’s Insight DRF manages the dynamic detector differently. Insight Agile acquires 100% of the data resulting in much less motion blur. Insight Agile motion blur at 7 fps is less than most systems at 30 fps as shown below. The images above and below were all acquired with the metronome operating at the same frequency.
